1. Complete the Varsity Medical Questionnaire with your medical history.
2. Print and take the Open Tryout Physician Form to your physician and ask them to complete it. Please note that any cost associated with obtaining medical clearance is at your expense.
3. Return the completed forms no less than 48 hours prior to your scheduled open tryout time to Kerri Downer (kdowner@mtroyal.ca).
